Simplified Issue: A Few Health Questions, No Exam
Simplified issue is the most common way final expense insurance is sold. Instead of a medical exam, you'll answer a short list of yes/no questions about specific conditions — things like recent hospitalizations, certain cancer diagnoses, or terminal illness.
Applicants with well-controlled, common conditions such as managed diabetes, high blood pressure, or high cholesterol are frequently still eligible, sometimes at a standard rate and sometimes at a modestly higher rate depending on the carrier's underwriting guidelines. Because there's no exam to schedule or lab results to wait on, approval decisions are often issued within a day or two.
Guaranteed Issue: No Health Questions at All
Guaranteed issue final expense insurance goes a step further and asks no health questions whatsoever. Nearly anyone within the eligible age range — typically 50 to 85 — is accepted automatically.
The tradeoff for that guaranteed acceptance is a graded (or modified) death benefit for roughly the first two to three years of the policy. If death occurs from natural causes during that window, the beneficiary typically receives all premiums paid back plus interest rather than the full face amount. Accidental death, however, is usually covered at the full face amount starting on day one.

Why Insurance Companies Can Skip the Exam
Final expense face amounts are intentionally small — usually $2,000 to $50,000 — which limits how much financial risk the insurer is taking on any single policy. That lower risk, combined with prescription history databases and other data sources insurers can check electronically, lets carriers substitute a short questionnaire (or nothing at all) for the exams required on larger traditional policies.
The result is a much faster underwriting process for both the applicant and the carrier, with decisions frequently made in minutes to a few days rather than the several weeks typical of fully underwritten policies.
Which Option Should You Choose?
The right underwriting path mostly comes down to your current health.
- ✓Reasonably healthy applicants: simplified issue usually offers the lowest premium for the same coverage amount
- ✓Applicants with more serious health conditions or a prior decline: guaranteed issue is designed specifically as a dependable fallback option
- ✓Unsure which you qualify for: a licensed agent can review your health history over the phone and recommend the better-priced option before you formally apply
